Chest pain on exertion

Why medical clarification is so important.

"I recommend that you have a cardiac catheterization as soon as possible."
"Oh no, it was so nice at your place."

This or something similar is what I hear in my (online) consultation hours. I'm happy when patients feel comfortable with me. But my job is to give honest advice, even if it's unpleasant.

Typical symptoms of coronary heart disease

Coronary heart disease (CHD) often manifests itself gradually and is therefore easily underestimated. Typical symptoms that should be taken seriously:


  • Chest pain or feeling of pressure on exertion ("angina pectoris")
  • Radiating pain in the arm, back, jaw or upper abdomen
  • Shortness of breath during physical exertion
  • Tightness in the chest or a "heart that stops beating faster"
  • Dizziness or unusual tiredness under stress
  • Feeling of pressure in the stomach area or between the shoulder blades

     

These symptoms usually occur during physical exertion and improve at rest. This is precisely a warning signal: the heart muscle is not getting enough oxygen at these times.

Caution: Diabetics, older people and women can often experience atypical symptoms such as  Upper abdominal pain, nausea, shortness of breath, severe palpitations and extreme tiredness!

Which examinations are useful

Indirect (non-invasive)

 

  • Myocardial scintigraphy
  • Stress echocardiography
  • Stress MRI

     

These methods check whether the heart muscle is receiving sufficient oxygen under stress. Their accuracy is around 80 %.


Coronary CT angiography

 

  • More suitable for low to medium risk.


Direct (invasive)

  • Cardiac catheterization
  • The coronary arteries are displayed directly here.
  • Accuracy: almost 100 %, but with a small risk of intervention.

Facts & figures - how often does CHD lead to a heart attack?

  • Coronary heart disease (CHD)) is the most common cardiovascular disease worldwide and a leading cause of heart attacks and deaths. In the USA alone, a heart attack occurs approximately every 40 seconds
  • More than 300,000 heart attacks occur in Germany every year, of which around one in five  asymptomatic (silent MI) progresses, especially in older patients or people with diabetes

Why there is sometimes no time for detours

For patients with typical stress pain and several risk factors, 20 % uncertainty is too high for me. That's why I recommend the direct route via cardiac catheterization in such cases.

In the end, however, the decision lies with the patients themselves.
My job as a cardiologist is to educate and advise without pushing.

My appeal

Exercise-induced chest pain is not a trivial symptom.
They must be clarified by a doctor in order to detect coronary heart disease at an early stage and reduce the risk of a heart attack.

FAQs on chest pain

If they occur during exercise/rest and may be associated with shortness of breath.

In angina pectoris, the blood flow in the coronary arteries is restricted; in a heart attack, a vessel is completely blocked.

A catheter is advanced to the coronary arteries via an artery in the wrist or groin. Using a contrast agent, constrictions can be visualized and, if necessary, widened with a stent (small wire tube = vessel support).  

Keymessage

Chest pain on exertion should always be clarified by a doctor. The earlier we recognize coronary heart disease, the better complications can be avoided.
